The Transforming Care After Treatment (TCAT) programme is a partnership between the Scottish Government, Macmillan Cancer Support, NHS Scotland and local authorities to support a redesign of care following active treatment of cancer.
Openbrolly Health is co-designing and delivering a solution with NHS Highland,using its ROC system, to collect symptom reporting, electronic holistic needs analysis and messages from cancer survivors. This information is combined with internal multidisciplinary team meeting summaries and other clinical data. The information is used to create a care plan, which is distributed to the service user and to primary and secondary care. In addition, the user gains access to a library of validated content tailored to their needs and concerns.
